Johns Creek Foundation Grant Application Documents

To be eligible for grant awards from the Johns Creek Foundation, the applicant must be a non-profit organization registered with the IRS under section 501(c)3. The organization must conduct exempt activities for the benefit of the citizens of the City of Johns Creek, Georgia. The organization must operate and provide the exempt activities within the City of Johns Creek, GA.

The receiving organization must use the grant funds awarded for the cultural, educational or recreational enrichment of the citizens of the City of Johns Creek. The funds awarded may be used to provide program services exempt under Section 501(c)3. The funds awarded may be used to acquire capital assets that will directly benefit the program services provided as noted above.


The Application Process

Applicants will be required to submit a letter request for the grant award to the Board of Directors of the Johns Creek Foundation. A narrative of the purpose of the grant request, a copy of the organization’s exempt status ruling under section 501(c)3, current financial statements, and two year budget forecasts must accompany the letter request. A copy of the most recent Federal and State compliance reporting documents, if applicable, must be presented with the letter request.  A copy of the organizations annual registration with the Secretary of State must also be provided.

Additionally, application will include:

1. List of Board of Directors including short bios.
2. Description of Organization (short history, including growth)
3. Description of who is served overall by the organization including specifics on Johns Creek residents (example, 75% students, etc.)

Project Specific Information

1. Project narrative, include any artists or organizations involved.
2. Dates and location of project.
3. Project Budget including other funding.
4. Identify specific need for funding from the Johns Creek Foundation including the exact amount requested and explanation of how it will be spent.
5. Proposal for Foundation recognition.

 

Recipient non-profit organizations will be required to provide periodic (quarterly) detailed reporting of the use of the funds provided the Foundation and attest to the exclusive use of these funds for qualified 501(c)3 purposes. Failure to perform the required reporting and attestation will result in the immediate request for reimbursement of the grant award. Grant awards must be used only for the purposes stated in the grant application and are approved exempt activities as defined by Section 501(c)3 of the Internal Revenue Code.

Prior to the award of the grant funds, the Foundation will require the execution of a grant contract with the recipient non-profit 501(c)3 organization. Among other terms, the recipient organization must attest to the exempt purpose of the use of the grant funds, and commit to comply with periodic reporting of the use of the grant funds to the Foundation.
